Q: Is Peak Balance Physical Therapy a registered Medicare provider?
A: Yes, Peak Balance Physical Therapy is a registered Medicare provider.

Q: Does Peak Balance Physical Therapy accept other health insurance companies?
A: Peak Balance Physical Therapy is currently out of network for other health insurance companies, however at the end of your visit you will be provided with a Statement of Insurance Reimbursement that you can submit to your insurance company.

Q: What is a Statement of Insurance Reimbursement?
A: A Statement of Insurance Reimbursement is a document provided by Peak Balance Physical Therapy at the end of your visit that contains information about the services you received and can be submitted to your insurance company for reimbursement.

REFERRALS

Q: Do I need a referral from a physician to be evaluated or treated at Peak Balance Physical Therapy?
A: No, a physician's referral is not required for evaluation or treatment at Peak Balance Physical Therapy.

SPECIALIZED SCOLIOSIS PHYSICAL THERAPY TREATMENT

Q: Why does scoliosis require specialized physical therapy treatment? 

A: Idiopathic Scoliosis is a condition that involves rotation and lateral flexion of the spine. Due to this, scoliosis needs to be treated in a 3-D approach. Please refer to the Scoliosis-Schroth Therapy section for more information on how this approach is beneficial to the patient.

SERVICE FEES

Q: How much do sessions at Peak Balance Physical Therapy cost?
A: Sessions are charged on an hourly basis at $175 per hour and $88 per 30 minutes.

Q: How long are evaluations and follow-up treatments?
A: Evaluations and follow-up in-person treatments are 1 hour long.

Q: Are telehealth sessions available?
A: Yes, follow-up telehealth sessions are available on a case-by-case basis and can be scheduled for 30 minutes or 1 hour depending on patient needs.

PAYMENT

Q: What payment methods do you accept?
A: Payment is accepted at time of service.  We accept major credit cards, apple pay, and google pay processed through Square for services rendered in-person. Telehealth appointments will be billed through our secure client portal and your transaction will be processed online after services are rendered. Additionally, after your first appointment is confirmed, you will have the option (not required) to store your credit card information in our secure client portal to automate payments.
